Before your first visit, please take the time to fill out our new patient questionnaire. Completion of the form will help prepare our staff and makes the check in process much easier.
On the online form, fields in yellow are required while fields in white are optional. Once every required field is completed, click “Continue” on the bottom right corner of the page (you may need to scroll off-screen if on mobile). When all pages are completed, make sure to click “Submit” on the bottom right corner of the final page.

What to expect at the first visit?
Our office goes above and beyond to make your first visit to Lana’s Dental Care as smooth and painless as possible. First appointments typically range from 60 – 90 minutes and will include the following services:
Digital panoramic x-ray: This image gives us a good view of all the soft and hard tissue of your oral cavity. It takes less than one minute to complete and only needs to be updated once every five years.
Digital bitewing x-rays: A set of bitewing x-rays, or “cavity detection” x-rays that allow the doctors to see in between your back teeth will be taken as well. This is the most common spots for cavities to develop. Bitewings include a total of 4 images and will be updated annually so that we can catch small cavities before they have a chance to develop into more serious conditions.
Photos: We also like to take your picture so that our staff can more easily identify you and your dental record. Please keep in mind patient photos are protected under HIPPA and cannot be shared outside of our office without patient consent. We may also take pictures of your teeth to aid the doctor during the exam or draw attention to something that isn’t easily seen on your x-rays.
Full-mouth charting of your gums: A vital component of checking for and monitoring gum disease and overall oral health is measuring “attachment loss.” We want to be sure that your teeth have the appropriate bone and tissue support to allow for proper oral health. We’ll take measurements for each tooth and record them in your patient file.
Comprehensive oral examination: One of our doctors will analyze the information we’ve gathered and perform an exam to get the most accurate picture of your oral health. We’ll then discuss our findings with you and go over what, if any, further treatment is suggested before your next cleaning. Our doctors will also address any questions or concerns you may have.
Thorough dental cleaning: Our qualified hygienist will finish up the appointment by scaling and polishing your teeth. If it has been a while since your last dental visit or you have some stage of gums disease, a more intensive gum treatment (commonly referred to as a deep cleaning) may be recommended to you. The doctor will determine this during the exam and discuss options with you if more advanced work is needed.
